Goosky RS4 Venom - noob - take-off assistance

Hello guys,
heli noob here. Had a couple failed take-offs with Goosky RS4 Venom, and looking for assistance.
What happened:
- heli spooling up on asphalt, -1deg collective pitch, other inputs zero-ed,
- heli starts bouncing a bit,
- i increase collective,
- heli moves forward, tips over forward, blades hit the asphalt.
Now, this is my second failed take off attempt. I can fly in AccuRC sim, but I think my heli is not well-set-up. (Detailed set up stuff below). Didn't record it but I want to "guess" what happened, for my next flight. I think I set the heli up pretty well, looking at vids from WestHobbiesRC. Could anyone answer these questions:
- should i be providing elevator "back" command upon take-off? The main shaft is "slightly forward facing", e.g. not vertical to ground, due to heli design. Or should I keep all commands at 0? I'm guessing it's "expected" to move up and forward on positive collective command, due to this. On the other hand, I _think_ the FBL unit "integrates" commands, so it might "overshoot" when the heli spools up enough? Don't know exactly how it works.
- could this have happened if the heli blades were not up-to-RPM, e.g. not fully spooled up? I may have gotten a bit scared.
- Should I keep a negative collective when spooling up, or 0deg, or what? I get "scared" when it starts bouncing around.
- On un-calibrated Goosky GTS FBL, should I be using "attitude mode" for initial take-off, or better to keep it off?
Additional info:
- checked blade pitch at 0 collective, it was ~ 0deg for both blades, when rotated towards tail boom,
- did NOT check blade pitch when perpendicular to (nose - tail) direction
- check gyro direction/balancing behavior, seemed OK,
- checked command direction, e.g. elevator and aileron (cyclic), collective, and tail blade direction.
- checked heli was "balanced" by gripping main blades perpendicular to body and lifting the heli up sideways.
- no slop in commands,
- did flybarless unit servo setup, servo arms are 90deg, no binding.
I'm kinda bummed since in all videos the heli seems to take off smoothly. Perhaps I just increased collective too fast.
